The section is divided according to the study dates offered. Each section will list the Proficiency Tests offered for that particular study date. Please check the box of the type of proficiency test needed under each study date.
If no proficiency test is ordered for a study date, please leave the section blank for that study date.
If a box is checked for a study date, please indicate the quantity of sets in the space provided below. For instance, if 4 ColiPTs are being ordered for May 6th, 2008, check the box marked "ColiPT" under the section for May 6th, 2008. Under the "Number of Sets for May 6th, 2008" box, type "4".
If more than one type of study is ordered for a study date, please specify the quantity of each type of proficiency test. (ex. "3 ColiPT, 1 HPC, 1 mFC")
